CCI 2024 Block Party

Bring the whole family out to CCI’s Fourth Annual Block Party. The festive block party will include food, prizes, music, and more!

Our Cause:
The Block Party encourages members of the community to take control of their health. We’ll highlight CCI’s services, including confidential and free resources. We’ll also include representation from other health, fitness, career, and education organizations in the community.

This will be CCI’s official farewell party to Franklin Park as we are moving to a new location. You don’t want to miss it! Come out and join us!

If you don't have a patient portal account, please let us know, and we'll be happy to help you set one up. We want to make it easy and convenient for you to access your appointment information.

    We want to make sure that our services are accessible to everyone, which is why we offer a sliding scale fee discount program (English / Spanish) based on your income and household size.
