A good relationship with your primary care team can help you stay healthy.

CCI’s primary care team supports the health of all patients. Your primary care team will identify and address your physical and behavioral health needs. We will help you get the care you need to feel your best.

Primary care is your first point of contact in the health system. You visit your primary care provider (PCP) when you get sick or need a check-up. They help you with annual physicals, vaccinations, treating minor illnesses and injuries, and monitoring chronic conditions.
Your PCP can also help guide your health and wellness journey, advising you on things like eating healthy food and staying active.
You may need to see a specialist to care for a more complex problem. Your PCP can help you find the right one.
You are the most important part of your care team.
CCI’s care team includes you. We want to make sure you feel able to make decisions about your health. Be curious, ask questions, and work with your care team to make a plan that works for you. Do your best to follow it.
