What to Know Before Your First Visit

Welcome to CCI Health Services. You are at the center of our patient-centered approach to care at CCI and the most important person on your care team. Together we’ll make sure you receive personalized, coordinated, and effective services.

Here’s what you can expect from us:
You will see qualified, caring, and courteous providers and staff.
We will work to get you an appointment as soon as possible.
We will refer you to a specialist if your condition requires it.
We will keep your health records private as ordered by law.
We will be honest with you about your health.
We will offer a sliding fee scale so that you can afford to see your care team, no matter what.
What we ask from you:
Be honest and open about your health concerns or problems.
Work with your care team to design a realistic care plan and follow the plan.
Be courteous to all our employees and fellow patients.
Be on time for appointments. Call ahead if you must cancel an appointment. If possible, give us 24-hour notice.
