We offer primary care at six (6) locations. As your Primary Care Home, we will identify and address your physical & behavioral health needs. Our team-based patient-centered approach to care for adults and children, includes routine visits, common laboratory tests, acute and chronic care, routine immunizations, diabetes education, nutrition counseling and referral to specialists.
Oral health services are integrated into five (5) of our primary care practices. We provide preventative and restorative services including routine exams, teeth cleanings, x-rays, composite fillings, sealants, fluoride varnishes, simple extractions, and prophylaxis with fluoride. Dental education is an integral part of our care delivery. After each visit, providers demonstrate proper brushing and flossing techniques; distribute multilingual, low-literacy educational materials, and offer patients take home dental supplies.
Behavioral Health Services
Behavioral Health Services are integrated into our primary care delivery and are offered at four (4) of our sites. Every medical visit starts with a short assessment tool used to identify depression and substance use. If the need arises, patients are internally refer to our certified behavioral health consultants. A patient can also request Behavioral Health Services. They provide individual, couple, and family counseling for ages 6-Adults
Pediatric Care services are available at five (5) of our primary care locations. Whether your child needs a checkup or vaccinations, we offer a range of services for children from infancy through young adulthood. During each routine well-child visit, a health care provider will check your child’s growth and development, in addition to providing insight on variety of issues for your child’s overall health and wellness. Integrating programming such as Weight Management and Behavioral Health, we ensure that your child receives comprehensive primary care to help keep her/him healthy at all stages of life.
Title X Family Planning services are integrated into all of our primary care practices. Through the program, we offer patients an array of reproductive and sexual health services, including gynecological care, STI/STD testing and treatment, a variety of contraception options, and pregnancy testing. In addition, we facilitate reproductive health seminars in a variety of community settings to educate its patient population about healthy sexual decision making and family planning options.
Established in 2014, this comprehensive prenatal practice is conveniently located in an area of Greenbelt, Prince George’s County, known as Franklin Park. It provides affordable, comprehensive, quality prenatal services that are tailored to the diverse cultural and ethnic backgrounds of women in the county. In 2015, CCI Health & Wellness Services adopted CenteringPregnancy as its standard of care. This is an innovative model of care that combines individual prenatal assessments with education and patient-driven group discussions.
The HIV and Infectious Disease team, based out of our Greenbelt practice, provides patients with comprehensive methods of disease control including vaccination, treatment programs, education, coping, disclosure, nutrition, and ongoing support for different infectious diseases. Our infectious disease specialists are well versed in the latest infectious disease, vaccine-preventable disease, and HIV research to best provide the support you need to live long and healthy lives.
WIC is a nutrition program that provides health education, healthy foods, breastfeeding support, and other services free of charge to pregnant and postpartum women, infants and children who qualify in Montgomery and Prince George’s Counties. CCI-WIC has five (5) convenient locations, extended and weekend hours to fit your schedule. When you come in for your appointment make sure to bring all family members applying for WIC services and the required documentation.
