If you, your family member, or someone you know needs health care, call the numbers below for an appointment, or stop by any location for information or to make an appointment at the front desk. CCI encourages appointments so you have reserved time to receive the attention you need. Walk-ins are welcome and will be seen as the schedule allows. Community Clinic services are available equally to all patients regardless of their ability to pay.** Tips for Your First Appointment **
Bring the following:
Picture ID
Insurance Card (If applicable)
If you do not have health insurance, you will be asked to pay on a sliding scale fee, which is based on your income and household information. Please bring the following documents with you:
Proof of Address:
Utility Bill (gas, water, electricity, or cell phone)
Or a notarized letter from your landlord
Proof of Income:
Two recent pay stubs or a notarized letter from your employer.
If you are not employed, you should bring proof of unemployment or a notarized letter from the person who supports you.
